If you do not upload a copy of your child's birth certificate to SportsConnect at least 2 weeks prior to your scheduled Skills Assessment, you MUST bring your child's birth certificate and medical waiver to the skills assessment. Birth Certificates will only need to be seen to verify ages, we will not be keeping them. All medical waivers will be turned in.
